Applicant must graduate from JHS this spring (2023) and plan to continue their education in college starting in the fall of 2023. If I am chosen as a recipient of this scholarship, I understand that in order to accept this award, I must attend a four year accredited college in the United States approved by the Jacksonville Education Foundation, or a Junior or Community College and be accepted into a Health Careers program (if applicable) before tuition monies are expended, up to $5,000 dependent on the type of program. A GPA of 3.0 must be maintained to renew each semester. The scholarship will be disbursed in the amounts of $2500 for semester 1, $1500 for semester 2, and $1000 for semester 3 with registration and GPA information submitted.
